Accidents were a minor factor in the casualty list. Our definition is deaths resulting from activities that were not directly associated with 'active service'. We have excluded Naval Accidents which are seperately identified because of their numbers and impact. Many accidents involved the aviators, operating at the the limits of technology.

Captain Thomas O'CONNOR, DSC, Royal Air Force He is listed as a Naval Officer (RAF) in the "Cross of Sacrifice" Vol. II He died on the 2nd September 1918 and is buried in Netley Military Cemetery, Hampshire, Grave Reference; Officers 1970 He was killed whilst flying in a Felixstowe F2a flying boat serial number N4414. Lieut B B Ball, Lieut J W Fox and Sgt W Robinson were all ingured (David Barnes) The Masonic Roll of Honour 1921 lists him serving in the Royal Navy. He was originally Commissioned in Royal Naval Air Service which by his death had been amalgamated within the RAF. It also lists that he was awarded the DSM; in fact it was the DSC.
